ONGOING CHRONIC
DISEASE MANAGEMENT
Our primary care physicians (PCP) and Family Nurse Practitioners (FNP) provides ongoing care for patients with chronic health conditions, such as heart disease, high blood pressure, diabetes or arthritis.
These conditions require regular monitoring and medication management. Our GloFusion providers can establish a relationship with you that continues over a long period of time in managing you care.
SCREENINGS & DIAGNOSTICS FOR COMPREHENSIVE CARE
Our practice also performs many diagnostic tests and general health screenings right in the office.
These includes pregnancy testing, hormone and thyroid tests, drug screening and cardiac diagnostics such as EKGs and arrhythmia testing, as well as general vision and hearing tests which can help to diagnose some health conditions. our primary care physician can also refer you to specialists if needed for other diagnostic tests if needed.
